A New Model for Chronic Care Management: A
Community-based "Public Utility" Part II
The second UCONNect webcast on April 20, 2009 featuring
panelists Victor Villagra, MD, Robert Zavoski, MD, MPH, Dean Sperry, Arman Ozgu, Patrick P. Coll, MD, and Robert Cushman, MD. The webcast was moderated by Thomas Agresta, MD. This webcast is a continuation of the first webcast
on March 30, 2009. Presentations and topics discussed included the C4 model from the information technology, transitional care, and medical homes perspective. Duration: 90 minutes.
Victor G. Villagra, M.D., F.A.C.P. is President of Health & Technology Vector, Inc. and an Assistant Professor of Medicine in the Ethel Donaghue Center for Translating Research into Practice and Policy at the University of Connecticut Health Center.
In the first UCONNect webcast he presented a novel approach to chronic care management based on
a public utility.
Thomas Agresta, M.D.
is an Associate Professor and Director of Medical
Informatics in the Department of Family Medicine at the UConn
Health Center. He also practices family medicine at the Asylum
Hill Family Medical Center in Hartford which is affiliated with
St. Francis Hospital and the UConn Health Center. Dr. Agresta will serve
as moderator for the UCONNect webcast.
Robert W. Zavoski, M.D., M.P.H. is the Medical Director in the Division of Health Care Administration of Connecticut’s Department of Social Services. Previously, he
has served as the Medical Director for Community Health Services, Inc. and
the Primary Care Center at Connecticut Children’s Medical Center. From 1993 through 2007, Dr. Zavoski served the Connecticut Chapter of the American Academy of Pediatrics in a variety of capacities, including two terms as
its President. In advocating for children and pediatricians in
the halls of state government, he
testified on over 75 pieces of legislation and spear-headed the
Academy’s successful advocacy of stricter teen-aged driving
laws, a ban on public smoking, and Connecticut’s Keep Kids Safe
license plate program.
Dean
Sperry is a founding partner of Equity Health Partners (EHP) a consulting and operating entity focused on strategic infrastructure change for the care delivery system. EHP delivers service management and heath IT solutions to successfully couple clinical quality improvement, operational efficiencies and value creation for healthcare provider organizations, health plans and employers.
Arman Ozgun is
co-founder and managing partner of Netsoft-USA, a technology services company headquartered in New York City. Mr. Ozgun has
over 15 years experience in Management and Technology Consulting, where he focuses on helping organizations improve
business performance via strategic process, organizational change and technology initiatives. His diverse background
includes process reengineering, strategy and operations, program management, IT governance, systems design and architecture
experience across a variety of industries including healthcare, financial services and supply chain. Prior to joining Netsoft
USA as a managing partner, Arman Ozgun has led complex enterprise transformation and technology projects for Ericsson,
Avon Products, Dannon and Allied Irish Bank. Arman Ozgun currently leads Netsoft USA’s healthcare initiatives and R&D
group. Mr. Ozgun holds a BS degree in Mathematics and Engineering.
Patrick
Coll, M.D., A.G.S.F. is a professor of Family Medicine and Medicine at the
University of Connecticut Health Center and is medical director
of the Seabury Retirement Community in Bloomfield, CT. He is a
diplomat of the American Board of Family Practice with a
certificate of added qualification in Geriatric Medicine. He is a
fellow of the American Geriatrics Society and a member of the
British Geriatrics Society. He is a member of the
Connecticut and American Academies of Family Medicine and is
listed in “The Best Doctors in America.” He will be
discussing Transitional Care.
Robert
A. Cushman, M.D. is Professor and Chairman of the Department
of Family Medicine at the University of Connecticut Health
Center and St. Francis Hospital & Medical Center. His interests include asthma, the doctor-patient-family relationship, community-oriented primary care, and international family medicine models.
He will be discussing achieving Medical Home Status as it
relates to the C4 Public Utility Proposal.
